Daily Medical Diary
If you have a medical condition or suspect a medical condition, you should keep a daily diary of your medical symptoms. Parents should keep one for their child(ren). This can be used to track the various symptoms and the conditions that each symptom happens under. This can give your medical doctor a better picture of what is happening with you medically. It becomes especially important when a new medication is introduced, the dosage is increased, or the dosage is decreased. In each entry, you should include the time, the date, the symptom, weather conditions, and what you were doing when the symptom occured. This should be done for each incidence during the day, evening, or night periods of the day even if more than one symptom happens in a day. This should be kept in a binder or notebook to make it easier to take to each doctor's appointment.

June 23, 2007 at 1:10 p.m. I was in my front yard pulling weeds out of my garden. I reached out for a weed, and I suddenly had a back spasm in my lower back. It was a sunny day with no clouds in the sky. Took some aspirin and the symptoms went away.
